Release of Claims and Indemnification Agreement // I am the parent or legal guardian of said minor ('Minor'). Prior to signing, I have read this Agreement in full, and I have fully informed myself of the risks associated with Minor's participation in Lincoln Christian School's Athletic Programs ('the Athletic Activity'). I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Oklahoma, and that if any portion of this Agreement is held invalid, the balance shall continue in full legal force and effect. I agree, individually and on behalf of Minor, to release and to hold harmless Willie George Ministries, Inc. d/b/a Church on the Move and Lincoln Christian School and their directors, officers, volunteers, and employees (collectively referred to as 'the Church and School') from any liability (including but not limited to liability for Minor's injury, death, or damage to or loss of Minor's personal property) resulting directly or indirectly from his/her participation in the Athletic Activity. I specifically release the Church and School from any liability resulting from the negligence of Minor or the Church and School. In other words, I will not sue the Church and School for any reason, whatsoever. I have consulted a physician regarding these risks and Minor's physical ability to participate in the Athletic Activity, or I have determined it unnecessary to do so, and on behalf of Minor I personally assume these risks. I personally assume all risks and liabilities in connection with Minor's participation in the Activity and agree to indemnify the Church and School against any liability assessed against the Church and School as a direct or indirect result of Minor's participation in the Athletic Activity. This release includes all risks and liabilities connected with the Athletic Activity, whether foreseen or unforeseen. In the event that Minor is injured during the Activity, and I am unable to provide consent to his or her medical treatment, I authorize the Church and School to consent on my behalf to the performance of any and all medical treatment judged necessary by them, until I am able to provide consent or until someone legally able to speak on Minor's behalf is made available. I agree, individually and on behalf of Minor, to release, indemnify, and hold the Church and School harmless from any liability which may be assessed against them as a direct or indirect result of said medical treatment. I agree to pay or arrange for payment for all costs associated with said medical treatment. *Lincoln Christian School requires all 7th through 12th grade prospective athletes to have a physical before participating in any sport. *Lincoln Christian School recommends that all students have a physical before participating in any sport, athletic in nature, event, or physical education classes.
